THE LONDON ORTHOPAEDIC CLINC, 79 WIMPOLE STREET, LONDON

I was invited to join the London Orthopaedic clinic as the first female Foot and Ankle Surgeon in the Harley Street area.  In addition to the conditions that I treat in my NHS practice, I am also developing a specialist interest in hypermobility and the Performing Arts.

The significant benefit of Private Practice as a Consultant, is having more time to spend with patients and be to understand how their condition or injury impacts on their life.  Being able to devlop a bespoke lifestyle service, which not only helps patients heal and recover, but also considers their work / life balance, is hugely rewarding.  It also gives me an opportunity to meet people from different backgrounds and cultures, which in itself is very satisfying.

I established an integrated multidisciplinary Foot and Ankle Service for NHS Lanarkshire.

 

Supervising the vetting of all Foot and Ankle referrals for NHS Lanarkshire

Development of Musculoskeletal pathway for Foot and Ankle

Development of patient information leaflets and post-operative protocols

Establishing agreed outcomes of assessment for conservative and surgical treatment of foot and ankle pathology

Training and education of medical, nursing and allied health professionals with regards to foot and ankle pathology

Established Virtual and Specialist Multidisciplinary Foot and Ankle Clinics

Development of Nurse Led Foot and Ankle Clinic

Management of Diabetic and Rheumatoid Foot and Ankle patients

 

I employ the most up-to-date techniques in Foot and Ankle Surgery, including: Minimally Invasive Surgical techniques, foot and ankle arthroscopy and soft tissue techniques, total ankle replacement and deformity correction (excluding Ilizarov / TSF).

I take referrals from all three Lanarkshire hospitals for complex foot and ankle trauma.

Management

 

LEAD CLINICIAN ORTHOPAEDICS – HAIRMYRES HOSPITAL                                                     2012 – 2014
This position enabled the development of the Orthopaedic and Trauma Service at Hairmyres Hospital and NHS Lanarkshire.

My first year focused on developing the Trauma service, which involved rota changes, establishment of a daily Trauma Meeting and development of an escalation policy for when demand outstrips capacity.

Monthly Orthopaedic Departmental Meetings were established, involving all representatives from all aspects of the outpatient and inpatient service, including medical, nursing, allied health professionals and the business manager for Orthopaedics, Orthopaedic Departmental Co-Coordinator and Head of the secretarial staff.  This group addressed all aspects of the patient journey from point of referral to discharge.  It encompassed outpatient clinics, pre-assessment, day surgery, inpatient wards and theatres.

As Lead Clinician for Orthopaedics I represented the department on the following committees:

The NHS Lanarkshire Orthopaedic reconfiguration group, was established following the Healthcare Inspection Scotland Report, which acknowledged concerns over the quality of care of Orthopaedic Services at Hairmyres Hospital, I was involved analyzing the rotas for the junior and middle grades, to attempt to provide appropriate overnight cover.

I was the representative for NHS Lanarkshire for the MSK Orthopaedic Quality Drive and ACCESS programme, for the Scottish Government.

 

BOFAS – SURGICAL PODIATRY SUB-COMMITTEE                                                                                                         2014 – 2015

 

Member of the BOFAS sub-committee, which is working with NES (National Education Scotland) to develop course, examination and appraisal standards equivalent to the FRCS Tr & Orth, to ensure that any course that is developed for the training of Podiatrists Practicing Surgical Podaitry, meet equivalence to the that of Orthopaedic Foot and Ankle Surgeons, who practice forefoot surgery.

As a Consultant I trained with Mr David Redfern (Brighton) and Dr Joel Venoir (Sussex and France) in Minimally Invasive Surgical techniques.  I have also trained extensively in Total Ankle Replacement. I have developed my surgical skills in arthroscopy and soft tissue reconstruction of the foot and ankle.

A number of patients require admission to the rehabilitation ward, as they are unable to non-weight-bear following ORIF of ankle fracture. Pain inhibition prevents patients fully weight-bearing until the fracture is healed.  Patients should be instructed to weight-bear immediately post operatively.

To confirm this we are undertaking a pilot study utilizing a pressure transducer to establish the amount of pressure taken by the injured limb at weekly intervals for the six-week period.

The Correlation Between Abnormal Psychopathology and Biochemical Markers Following Orthopaedic Trauma

Case-controlled pilot study to determine an association between biochemical markers and the physical and psychological aspects of trauma.  The fundamental markers of the inflammatory response, (IL-6, IL-6r and CRP), were compared with symptoms of PTSD.

 

This pilot study showed positive correlation between some of the symptoms of PTSD, particularly intrusive phenomena and the acute phase inflammatory markers.  This work has been published and subsequently formed the basis of a subsequent larger study.

There is little in the literature with regards to the blood supply to the navicular.  We are undertaking an anatomical study to determine the blood supply to the navicular, which may explain the clinical observation that the lateral aspect is more commonly affected by avascular necrosis or osteochondritis.

Career Aims

Having effective established an integrated multidisciplinary Foot and Ankle Service in NHS Lanarkshire my focus is now on establishing a Private Practice based in London at the London Orthopaedic Clinic and Princes Grace Hopsital.

My main interests within Foot and Ankle Surgery are the continued development of Minimally Invasive Foot and Ankle procedures, which includes diabetic foot pathology and complex deformity correction.  I also intend to further develop my foot and ankle Sports pathology, including arthroscopic procedures.  I continue to develop my total ankle arthroplatsy practice, including outcomes associated with the procedure.

I am very interested in teaching and training, including GP’s, Physicians and other Orthopaedic Surgeons.  I also enjoy working closely with allied health professionals and will continue to develop these links and close working practices.
